The blood work moves first. Most men expect to feel it and then check it; for a meaningful share, checking was the only way they would have known at all (Araujo et al., Journal of Clinical Endocrinology & Metabolism, 2007 — Boston Area Community Health Survey, n=1,475).

A threshold is a definition, not a target to hit. It is a reason for a physician to look further, read alongside the rest of the panel (Araujo et al., 2007).

A standard annual physical checks total testosterone only if someone asks. Free testosterone almost never appears at all.

In a 2025 UK survey of 973 men recruited online, 49% screened symptomatic on a self-report questionnaire while 5% had ever been formally diagnosed. That survey used a different instrument and population than the Boston cohort, and its authors are affiliated with a commercial men’s health company — it is reported here as a self-report screen, not as a prevalence figure (Liu et al., BMJ Open, 2025).

What We Will Not Tell You

Every claim we make ships with its limit attached. Here are the four we will not.

We will not tell you this is low testosterone.

A questionnaire is a screen, not a diagnosis, and a threshold is a definition, not a target. Nothing here diagnoses anything. That is what a physician-supervised panel is for.

We will not sell you testosterone therapy.

In a meta-analysis of four randomized trials (n=1,779), testosterone replacement produced small improvements in sexual desire, erectile function and satisfaction — and no effect on energy or mood — alongside an eightfold increase in erythrocytosis risk, on a very wide confidence interval (Ponce et al., JCEM, 2018). Kassy Wellness does not sell it.

We will not promise that measuring changes an outcome.

There is no randomized trial showing that the measurement path described here changes a clinical outcome. Until there is, we will not imply one. What it produces is an answer where there was a guess.

We will not tell you a general health check saves lives.

Cochrane’s systematic reviews of general health checks in adults have not found that they reduce mortality. That is a real finding about a real practice — the undirected annual physical, ordered for everyone. What is described here is narrower: one specific question, asked with a panel chosen for it.
